Effective March 8, 2013, employers should begin to utilize the updated Family Medical Leave Act (FMLA) forms (revised February 2013) released by the Department of Labor (DOL), when applicable. The updated forms are: FMLA Form WH-381 (Notice of Eligibility and Rights & Responsibilities), FMLA Form WH-384 (Certification of Qualifying Exigency for Military Family Leave), FMLA Form WH-385 (Certification for Serious Injury or Illness of Current Servicemember for Military Family Leave) and FMLA Form WH-385-V (Certification for Serious Injury or Illness of a Veteran for Military Caregiver Leave).
